Additional Medical Information
Imaging Studies
If you have completed any recent imaging studies such as a CT or MRI, we strongly encourage you to bring these with you to your appointment for review. Failure to do so may require us to schedule an additional appointment. Please contact us with any questions regarding imaging.
- MRIs: When you complete an MRI study, please ask for a copy on CD to bring with you to your next appointment.
Pertinent Medical Records:
Please alo bring any recent medical records (within the past 5 years) related to the medical condition you are being treated for today.
- Operative notes from previous surgeries
- Discharge summaries from ER visits or recent hospital stays.
- List of current medical problems and medications you currently take.
